抄録: A 32-year-old man associated with factor Ⅶ deficiency and IgA nephropathy is reported. According to his family history, the patient's parents were cousins in marriage and his younger brother was also affected by factor Ⅶ deficiency. Twenty-five years ago, moderate proteinuria (1-3 g/day) was first discoverd. A renal biopsy specimen showed mild to moderate mesangial proliferative glomerulonephritis with IgA deposits. Hemostatic tests demonstrated typical factor Ⅶ deficiency : marked prolongation of coagulation time, partial thromboplastin time and absence of factor Ⅶ. In his past clinical course he had had a transient cerebrovascular attack presenting with left hemiparesis 15 years ago. At last observation, his renal function remained normal although moderate proteinuria persisted. To the author's knowledge, this is the first reported case associated with these two diseases.</description>

抄録: To minimize various post-transfusion complications caused by white blood cell components, Sepacell R-500 A filters (Asahi Medical) for red blood cell products and PL 50/100 filters (Pall) for platelet products to remove white blood cells were clinically&#xD;
used in a total of 46 patients with leukemia and its allied disorders, and the results were compared with those of 19 patients with whom the filters were not used. Platelet allo-antibodies, assayed by mixed passive haemagglutination (MPHA) method, developed in 10 patients out of 19 (52.6%) without use of the filters and in 8 patients out of 22 (36.4%) with whom the filters were partially used. However, development of such antibodies was significantly lower when pre-filtered blood products were used throughout ; only two patients out of 24 (8.3%). Another common complication, a non-hemolytic febrile reaction, was also considerably reduced by using these filters. Therefore, we recommend that these filters should be used from the beginning of transfusion therapy, and that their use should never be interrupted throughout the treatment.</description>

抄録: To evaluate the severity of diabetic renal involvement, we performed electrophoretic analysis of urinary glycoprotein for 16 patients with diabetic nephropathy using peroxidase-lectins. After being subjected to SDS-polyacrylamide gel electrophoresis, urinary glycoprotein was transferred to the nitrocellulose membrane, which then was stained with peroxidase labeled-lectin (Triticum vulgaris ; WGA), and was analyzed&#xD;
densitometrically. Electrophoretic analysis of urinary glycoprotein revealed the presence of lectin-reactive glycoproteins with the molecular weights of 22 kD, 32 kD, 38 kD and 48 kD respectively. Among these, the amount of the 22 kD-mol. wt. glycoprotein increased significantly as the renal histopathological changes developed, whereas that of 32 kD decreased. These results suggest that semiquantitative analysis of low molecular weight urinary glycoprotein stained with lectin is useful for the non-invasive evaluation of the severity of diabetic nephropathy.</description>
