抄録: We report a rare case of adult intussusception caused by an inverted
Meckel's diverticulum. A 24-year-old man with intermittent abdominal pain was found
by abdominal computed tomography to have target sign. We diagnosed intussusception,
and couducted emergency surgery. At laparotomy, we found that the ileum had
invaginated into the ascending colon. When the intussusception was relieved with
Hutchinson's maneuver, an inverted diverticulum was observed at 80cm proximal to the
ileocecum. The diverticulum was 7cm in size and was present on the opposite site of the
mesentery. Histological findings showed all layers of small intestine and ectopic gastric
mucosal tissue in the diverticulum. We diagnosed intussusception due to inverted Meckel'
s diverticulum. Inverted Meckel's diverticulum should be considered as a probable cause
of intestinal intussusception.

肺癌の病期診断と治療方針決定におけるFDG-PETの有用性の検討

タイトル: 肺癌の病期診断と治療方針決定におけるFDG-PETの有用性の検討

抄録: CT images and FDG-PET images of 31 cases from 30 patients with
histologically proven lung cancer were reviewed. The clinical stages and treatments
determined by CT images alone (CT-stages and CT-treatments) were compared with
those by CT and FDG-PET images (PET-stages and PET-treatments). The CT-stages
were different from the PET-stages in 11 cases (35%). The CT-treatments were different
from the PET-treatments in 10 cases (32%). These results suggest that the additional use
of FDG-PET to the standard clinical staging by CT may have a great influence on the
choice of optimal treatment strategies for lung cancer.

特別養護老人ホーム入所高齢者の生体リズムの特徴とその関連要因

抄録: It is universally recognized that biological rhythms in the frail elderly with
disorders such as dementia are liable to disruptions; however, studies exploring the
characteristic features of biological rhytims in the elderly and the factors causing
disruptions of the rhytims in the frail elderly, especially those in need of care and
residing in nursing homes are still few. We conducted the present study to examine the
characteristics of the biological rhytlms and the factors causing disruptions of the
rhythms in 50 residents of special nursing homes for the elderly (9 men and 41 women)
using a contmuous portable instrument. The results revealed disrupted 24-hr rhytims in
22 subjects (44%), nearly half of the study subjects. The results also suggested the
possible involvement of wake-sleep cycle reversal (P = 0.032) in the rhythm disruptions.
Activitles of daily living (P = 0.046), poriomania (P = 0.039), and participation in actions
(P = 0.039) were also found to be significant factors contributing to disruptions of the
biological rhytlms. Seventeen residents (34%) showed considerable decrease in the
activities of daily living. The present findings on the characteristics of biological
rhythms and the factors involved in causing the disruptions of the rhythms in elderly
nursing home residents provide basic information for considering interventions to
improve care for the frail elderly in need of care in the clinical practice setting of nursing
care for the aged.
