著者: 大澤, 弘吉
抄録: The present study was designed to elucidate the manner of atrial natriuretic peptide (ANP) and vasopressin (AVP) secretion, which involves the development of water intoxication in patients with chronic psychosis. Plasma ANP level and plasma AVP
level to plasma osmolality were measured before and after oral water loading in fifteen psychotic patients and in fifteen normal controls. 1) In the patient group, plasma AVP was detected even below 270 mOsm/kg. Sensitivity of AVP secretion to osmolality was lower in the patient group than in the control group. 2) The level of ANP was higher in the patient group than in the control group before and after water loading. There was a negative exponential correlation between ANP level and plasma osmolality in the patient group. 3) From these results, we infer that water intoxication in schizophrenic patients might be 'hypovasopressinemic antidiuresis' (one type of SIADH), linked with ANP secretion.

抄録: The case of a 6-year-old girl infected with Diphyllobothrium latum is described. About 3 months previously, she had eaten sashimi (sliced raw fish) of salmon, the second intermediate host of D. latum. On the day before treatment, food intake was restricted and she was then treated with a single administration of 600 mg of praziquantel (30 mg/kg). After administration of a laxative and an enema, the moving worm of D. latum with scolex was discharged with the stool. There were no side effects during treatment. Three months after treatment, stool examination did not reveal any parasite
eggs in the stool. Treatment with praziquantel could be one of the more satisfactory and safer anthelmintic methods in child patients with D. latum infection.

著者: Yajima, Hiroshi; Tamai, Susumu; Mizumoto, Shigeru; Kawanishi, Koichi; Ishida, Hitoshi
抄録: We experienced a case in which ectopic bone formation with trabecular structures occurred after free peroneal flap transfer containing periosteum of a fibula onto a soft tissue defect on the dorsum of a foot in a 2-year 9-month-old boy. In this case, ectopic bone formation was detected by radiography 5 months after the operation and the bone was removed because of the restricted dorsiflexion of the ankle. This case suggests that new bone can be formed by the periosteum if it is grafted with its vasculature preserved by microvascular anastomosis.

タイトル: 自然発症糖尿病ラット(WBN/Kob)における心筋障害とカテコラミン代謝に関する研究
著者: 加藤, 茂
抄録: To clarify the relationship between cardiac involvement in diabetes and catecholamime metabolism, hearts of the male WBN/Kob rat, a new strain of spontaneously diabetic rat, were examined morphologically, and catecholamine concentration in cardiac tissue was measured. The heart in WBN/Kob rats showed focal myocytic necrosis and replacement fibrosis of the myocardium, and hypertrophy and dilatation in the left ventricle at the advanced stage. There were no significant morphological differences in the arteries and arterioles between WBN/Kob and control rats. In WBN/Kob rats, low catecholamine concentrations in cardiac tissue and high sensitivity of heart rate to catecholamine were present before the manifestation of diabetes, compared with control rats. To evaluate the effects of β blockade or angiotensin converting enzyme inhibitor on cardiovascular lesion, β blockade (metoprolol ; 0.4 mg/100 g BW/day) or ACE inhibitor (captopril ; 0.2 mg/100 g BW/day) was administrated to WBN/Kob rats for 18 months. In WBN/Kob rats treated with metoprolol and captopril, the systolic blood pressure was lower than that of controls (non-medicated WBN/Kob rats), and the heart weight as well as cross-sectional area of the vestricular wall were smaller than those of controls. In WBN/Kob rats treated with metoprolol, the degree of dilatation and fibrosis area of the left ventricle was diminished compared with that of other groups of rats. β blockade prevented the cardiovascular involvement in WBN/Kob rat from progression. The present study suggests that high sensitivity of cardiomyocyte to catecholamine may lead to cardiovascular involvements in diabetes mellitus.
