抄録: A 56-year-old man who had received bone marrow transplantation under the&#xD;
diagnosis of Hodgkin's lymphoma (Stage ⅣB) complained of tarry stool during of&#xD;
immunosuppresslve agent theropy. Upper gastrointestinal endoscopy revealed longitudinal ulcers in the second portion of the duodenum. Histological examination of the ulcer&#xD;
margin showed intranuclear inclusion bodies. Although we treated the patient with anti-&#xD;
CMV agent and proton pump inhibitor, he complained of tarry stool again. Upper&#xD;
gastrointestinal endoscopy revealed Dieulafoy's lesion in the superior duodenal angulus.&#xD;
However, it was difficult to observe this lesion by the rigid endoscope; we were able to&#xD;
observe and treat the condition by using endoscopic clipping with a transparent hood.</summary>

    <title>地域リハビリテーションチーム医療の連携</title>

抄録: To perform successful physical rehabilitation, it is usual that home training&#xD;
outside the care and rehabilitation facility is also needed. However, when a therapist&#xD;
only contributes to teaching the users of the care and rehabilitation facility to perform&#xD;
home training without support by staffs of comprehensive medical care and caregivers,&#xD;
the users are passive, resulting in less success in physical amelioration. Effects of a&#xD;
team approach by staffs of comprehensive medical care and caregivers on rehabilitation&#xD;
performance in the care and rehabilitation facility were explored. Aim: The subiects of&#xD;
this study were 40 rehabilitation vsers using a daycare center for 3 months. Methods:&#xD;
Using Functional Independence Measure (FIM), the difference in the scores between&#xD;
before and 3 months after the team approach was evaluated. Results: The overall score in&#xD;
January was 3594 points and that in April was 4396 points; and the difference was 802&#xD;
points, indicating the increase of the score. Mean score per user in January was 89.9&#xD;
points and that in April was 109.9 points, with a difference of 20.1 points, indicating the&#xD;
increase of the score per user. Discussion: The present results indicate that the team&#xD;
approach improved physical amelioration. Thus, a therapist should organize the team&#xD;
approach with staffs of comprehensive medical care and caregivers around the user. As a&#xD;
resvit, the users of the care and rehabilitation facility will be able to actively perform&#xD;
home training in their homes with support by staffs of comprehensive medical care and&#xD;
caregivers, leading to progress in the quality of life of the users and their families.</summary>

抄録: Among the problems in today's medical care that we should consider are long&#xD;
waiting times and short office hours. Under these circumstances, the pathological state&#xD;
of the patient with Parkinson's disease easily changes during everyday life, even though&#xD;
periodical consultations with a doctor are carriedout. At present in medical institutions,&#xD;
medical exalnination and consultation hours are largely restricted and thus sufficient&#xD;
medical interrentions are not performed for this intractable disease. The aim of the&#xD;
present study was to develop an evaluation standard list composied of items&#xD;
corresponding to activities of daily living (ADL) specific for Parkinson's disease1) and&#xD;
quality of life (QOL)2) in order to understand psycho-physical function and ADL, and to&#xD;
arrive at a prognosis during a short medical examination by interview. The subjects for&#xD;
this study were 50 patients diagnosed with Parkinson's disease. I made a subjective QOL&#xD;
questionnaire and examined the subjective satisfaction of patients to their own QOL&#xD;
using this questionnaire. The items tightly correlated with satisfaction in daily life were&#xD;
falnily, role inthe home, relatives, costs of living, and satisfaction with house in home&#xD;
level. Using these tightly correlated items, an evaluation standard list that enables the&#xD;
shortening of appropriate medical examinations and treatments will be developed.</summary>

    <summary type="text">タイトル: 天理市立病院における小児スポーツ眼外傷の発生状況

抄録: Among patients who visited the Ophthalmological Department of Tenri&#xD;
Municipal Hospital due to sports-related eye injuries between July 1997 and March 2006,&#xD;
children aged ≦ 15 years were studied retrospectively. The evaluation items were age,&#xD;
gender, sport that induced injury, cause of injury, and ocular findings at the initial&#xD;
consultation. In patients with a corrected visual acuity of ≦ 0.6, visual acuity was&#xD;
surveyed at the final observation as well as ocular findings. The subjects were 122&#xD;
children with unilateral eye injury. The age at the time of injury was most frequently 13&#xD;
years. The sport that most frequently induced injury was baseball, with injuries due to&#xD;
balls observed very frequently. At the time of the first consultation, 6 of the 122 patients&#xD;
had a corrected visual acuity of ≦ 0.5, and the other patients had a corrected visual&#xD;
acuity of &gt; 0.6. The ocular finding most frequently observed was traumatic iritis; in&#xD;
baseball and football, injuries tra㎜atic macula hole occurred in 1 patient each.&#xD;
Ophthalmologists should not only perform treatment of injuries but also give instructions&#xD;
and education to sports instructors and athletes as a part of concrete measures to reduce&#xD;
the incidence of sports-related eye injuries and, in additoin, develop methods of&#xD;
preventing the aggravation of injuries that have unfortunately occurred.</summary>
