抄録: A 69-year-old woman was admitted to our hospital because of gingival
bleeding on May 1997. She had been our outpatient for treatment of angina pectoris since
1987. She had had stomatorrhagia and slight fever since April 1997. On the present
admission, she had normocytic normochromic anemia and myeloblast with Auer bodies in
her peripheral blood. She also had dysplasia in her marrow cells. She was diagnosed as
having MDS (RAEB-T). Although MDS is usually associated with pancytopenia, this
patient had remarkable thrombocytosis. The 5q-syndrome sometimes has thrombocytosis.
However, her chromosome analysis showed normal karyotype, and she did not have
chromosome aberration. Thrombopoietin and IL-6 which are known as growth factors of
platelet did not increase. The present case is a rare case of MDS with remarkable
thrombocytosis. Her thrombocytosis may have been caused by neoplastic increase of
megakaryocyte and platelet in bone marrow.

著者: Zhu, Jiebin; Nakamura, Shinobu
抄録: Recently, it has been reported that arsenic trioxide (As2 O3) is an effective
anticancer agent for acute promyelocytic leukemia (APL). In the present study, we
examined the anticancer effects of As2 O3 at low concentration (0.25～2.OμM) on two hunan
leukemia/lymphoma cell lines, HL-60 and RL, in vitro. We found that As2 O3 inhibited the
growth of HL-60 and RL similar to the reported APL cell line, NB4. Typical apoptosis
was observed in morphologlcal study and DNA fragmentation assay, as well as a cell
cycle arrest at subG1. To address the mechanism of As2 03-induced apoptosis, we also
examined the effect of As2 O3 on the CD95/CD95L pathway and bcl-2 protein expression.
The results showed that the CD95/CD95L expressions were upregulated; meanwhile,
caspase 8 and caspase 3 were activated. However, the bcl-2 protein expression was
downregulated. Using anti-CD95 monoclonal antibody to block the CD95　pathway,
As2 O3 - induced apoptosis was ameliorated. These data suggest that in HL-60 and RL cell
lines the CD95/CD95L pathway and downregulation of bc1-2 protein expression are
invoIved in As2 O3-induced apoptosis.

抄録: Clinical observations were performed during the first 5 years (1996.7-2001.7)
of new patients at the Department of Oral and Maxillofacial Surgery, Takai Hospital. The
followlng results were obtained.
1. The total number of new patients was 4,080. The male-female ratio was 1:1. Many of
the patients were in their 20s. The youngest patient was 6 months old and the oldest
 patient was 100 years old.
2. The yearly average number of new patients was 780. 1,844 patients (45.2％) had
diseases associated with oral and maxillofacial surgery, and 2,236 patients (54.8％) were
 dentistry.
3. More than 75％ of the patients resided in Nara City, Tenri City, and Yamatokoriyama
 City.
4. 974 patients (69.7％) who had diseases associated with oral and maxillofacial surgery
were referred from dental offices, within the hospital and other hospitals. The number
of patients introduced to our hospital year by year.
5. 1,118 patients (27.4％) were medically compromised patients, and these patients were
almost all more than 60 years old. Of medically compromised patients, hyper tension
was found in 510 patients (36.9％).
6. The number of inpatients who underwent an operation in the central room was 272
cases, and mainly resulting from maxillofacial trauma. The number of in out patients
was 967 cases, and most of them receiving extraction of impacted third molar.

タイトル: Work-Related Musculoskeletal Disorders and Associated Factors in Teachers of Physically and Intellectually Disabled Pupils
著者: Yamamoto, Nobuhiro; Saeki, Keigo; Kurumatani, Norio
抄録: Teachers in schools for physically and intellectually disabled pupils are assumed to have a risk of developing usculoskeletal disorders including the cervicobrachial and lower back regions. To date, however, studies concerning these issues are extremely limited. The present study aimed at clarifying the magnitude of the musculoskeletal disorders among the teachers, its work-relatedness and associated factors. First, we conducted self-administered questionnaires and medical checkups simultaneously for 126 female nursery school nurses to ascertain the validity of questions and answer options which we developed for epidemiologic research. Questions "haveyou
had any pain or stiffness in your neck, shoulders or arms in the past month?" and "have you had any pain or stiffness in your lower back?", and an answer option "almost all day, everyday" were found to correlate well with the results of clinical examinations and difficulties in daily activities. Secondly, we analyzed self-administered questionnaires of
975 teachers working for physically and intellectually disabled pupils at municipal
schools in a city. The questions and answer options which had been validated in the first study were used. In the males 16％ and 20％ complained "almost all day, everyday" of the cervicobrachial and lower back symptoms, respectively, and in the females 20％ and 16％ in that order. Adjusted odds ratios of both symptoms increased with an increase in objective scores which were obtained by the homogeneity analysis and were considered to reflect the quantity of nursing care burden on the teachers. In case of the cervicobrachial symptom females against males had a significantly higher odds ratio than unity. These results demonstrated that the musculoskeletal symptoms complained of highly by the teachers were work-related, and that the question and answer options we developed could be useful parameters in epidemiologic research for the symptoms now widely prevailing in various workplaces.
