著者: Tohno, Setsuko; Azuma, Cho; Tohno, Yoshiyuki; Sano, Risa; Sawada, Yuta; Shimizu, AKihiro; Jimura, Hironobu
抄録: A case of incomplete double renal pelves and ureters was found in a 78-year-
old Japanese woman during an ordinary dissection by medical students at Nara Medical
University in 2007. The double renal pelves and ureters were present in the left kidney.
After two ureters were combined at the distal site, the combined ureter entered the
urinary bladder. The number of renal calices was five and three in the upper and lower
renal pelves of the left kidney, respectively. Furthermore, the right kidney and ureter
were normal. The double superior venae cavae were also present in the woman.

抄録: Two cases of sarcomatous HCC and CCC are reported. One patient, a 72-year
old female, had a 3.5 cm tumor in the lateral segment of the liver. The other, a 64-year
old male, had a 5.5 cm tumor in the anterior to posterior segment of the liver. Both
tumors were surgically removed. Histologically, both tumors were mainly composed of
atypical spindle-shaped sarcomatous or pleomorphic cells arranged in bundles and/or
sheets, together with a minor component of carcinoma elements of CCC in the former case
and HCC in the latter. Abundant inflammatory cell infiltration was observed in both
tumors. There were transitional zones between the carcinomatous and sarcomatous
elements in both cases. Immunohistochemically, sarcomatous cells of the former were
positive for vimentin, CAM5.2, CK7, CK19, and AE1/AE3, but negative for hepatocyte
specific antigen (HSA), anaplastic lymphoma kinase (ALK), α-SMLA, desmin and S100.
Finally a diagnosis of sarcomatous CCC was made. Sarcomatous cells of the latter were
positive for vimentin, CAM5.2, AE1/AE3, and CK18, but were negative for CK7, CK19,
CK20, HSA, AFP, ALK, α-SMA, desmin, and S100, and were finally diagnosed as a
sarcomatous HCC.

著者: 柴田, 真理; 山田, 佳世; 吉岡, 章; 嶋, 緑倫; 道之前, 八重; 高橋, 幸博
抄録: We report the case of a ten-month-old male, the second baby of twins. He
was delivered by caesarian section at a gestational age of 28 weeks and 5 days because of
maternal infection and uncontrolled tocolysis. His birth weight was 1,390g. Post-natal
respiratory care was required for respiratory distress syndrome. Group B Streptococcal
(GBS) sepsis was diagnosed at birth and the patient was treated intensively with
antibiotics. Cranial CT and magnetic resonance examination indicated irregularities
consistent with periventricular leukomalacia (PVL) on the right anterior lateral horn and
in the periventricular area. He had a febrile convulsion at the age of ten months, and
bilateral parieto-occipital multi-spikes and hypsarrhyttmia were evident on electroen-
cephalogram (EEG) 10 days after the seizure. Valpronic acid (VPA) was prescribed for
the prevention of attacks, but further infantile spasms occurred after 5 days. Convulsions
increased with series formation, and retardation of development was also noted. He was
admitted to our hospital with the diagnosis of West syndrome. Increased doses of VPA
and high doses of vitamin B6 (VB6) were administered without clinical benefit. In view
of the clinical deterioration, Zonisamide (ZNS) was additionally prescribed. Eleven days
after the initiation of ZNS treatment, the spasms disappeared, and the hypsarhytlmia and
irregular spike and wave complex patterns on EEG were resolved. The effects of therapy
with Zonisamide in West syndrome following PVL deserve wider discussion.

著者: Wang, Wenchao; Sawada, Masayuki; Ikeshita, Katsumi; Negoro, Hideki; Kishimoto, Toshifumi
抄録: The present study examined whether Tai Chi exercise had treatment effects
for major depression using objective measures. We used mismatch negativity (MMN) and
early negative difference (Nde), one of event-related potentials (ERPs) as objective
measures. We administered 24-style Tai Chi exercise twice a week during 3 months to a
patient with major depression. Consequently, not only depressive symptoms but also
small MMN and Nde amplitudes improved accompanying 24-style Tai Chi exercise.
Therefore, Tai Chi could be recommended as a mental training method to ameliorate
cognitive function, especially attention function.
