抄録: Two cases of heterotopic sebaceous glands in the esophagus, found in a &#xD;
71-year-old male and a 50-year-old female patient, are reported. In both cases, &#xD;
endoscopic examination revealed multiple yellowish plaques sized 1 to 3mm in diameter &#xD;
in the esophagus, which were initially interpreted as probable candida infection, &#xD;
xanthoma, or glycogenic acanthosis. Histological examinations of biopsy specimens &#xD;
showed clusters of clear cells in the squamous cell epithelium. They had pale cytoplasm &#xD;
with centrally located small nuclei and closely resembled sebaceous glands of the skin, &#xD;
and were histologically interpreted as heterotopic sebaceous glands of the esophagus. &#xD;
Along with recent advances in endoscopic technique, reported cases of heterotopic &#xD;
sebaceous glands in the esophagus are increasing. However, the histogenesis of &#xD;
heterotopic sebaceous glands in the esophagus has not been well established and remains &#xD;
to be established. The present report concerns two cases of heterotopic esophageal &#xD;
sebaceous glands accompanied by some immunohistochemical studies.</summary>

抄録: Purpose : Bladder carcinoma often occurs in the elderly. We compared the clinical results &#xD;
of radical cystectomy between elder and younger patients. &#xD;
Materials and Methods : We retrospectively reviewed the records of 59 patients who &#xD;
underwent radical cystectomy for bladder carcinoma between 1993 and 2006. Two groups &#xD;
were compared:≦75 years old (n=49) and ＞75 years old (n=10). Overall survival and &#xD;
disease specific survival were evaluated using Kaplan-Meier methods and a multivariate &#xD;
Cox model.&#xD;
Results : There were no significant differences in blood loss and transfusion volume &#xD;
during the operation. However, the operative time in the younger patients was longer &#xD;
than in the elderly patients (p＜0.01). The rate of complication in the elderly patients was &#xD;
not different from that in the younger patients (p=0.86). The 5-year disease specific &#xD;
survival rates for patients ≦75 and ＞75 years old were 80.1% and 42.5%, respectively &#xD;
(p＜0.01). The elderly did not receive adjuvant chemotherapy.&#xD;
Conclusions : In the current series, almost all the elderly patients tolerated radical &#xD;
cystectomy compared with the younger patients. Further work is needed to determine&#xD;
indications for radical cystectomy and to develop strategies to improve cancer control in&#xD;
elderly patients.</summary>

抄録: 近年,ヒト胚性幹細胞(ES細胞)からの様々な細胞への分化の報告がなされてお&#xD;
り,骨髄細胞由来の問葉系幹細胞なども樹立されて,再生医療への応用が期待されている.さ&#xD;
らに最近では,ヒト皮膚線維芽細胞からヒト人工多能性幹細胞(ips細胞)などの樹立もあり,倫&#xD;
理的問題がなく,拒絶のない夢の再生医療にさらなる注目がなされている.これらの未分化幹&#xD;
細胞から臓器特異的な細胞への分化は,胎児での胚形成と非常に似た分化段階を有しているこ&#xD;
とが分かっており,まず第1段階として,幹細胞からの3胚葉分化のメカニズムの解明が非常&#xD;
に重要である.これまで,ES細胞から外胚葉,中胚葉細胞への報告は多数あったが,内胚葉細&#xD;
胞への分化についての報告はなかった.我々は,無血清培地条件下でアクチビンによる内胚葉&#xD;
分化の方法を確立し,報告している.本稿では,我々のES細胞からの3胚葉分化についての研&#xD;
究成果と最近の知見について述べてみたい.</summary>
