抄録: Telomerase activities in hyperplastic nodules and hepatocellular car- &#xD;
cinomas (HCCs) induced by a choline deficient L-amino acid defined (CDAA) diet were &#xD;
examined by TRAP assay in rats. Male F 334 rats, 6 weeks old, received a CDAA diet for &#xD;
10, 50 or 75 weeks. Hyperplastic nodules and HCCs were noted in rats fed a CDAA diet for &#xD;
50 and 75 weeks, respectively. As a control, normal livers from 6 week old rats were &#xD;
obtained. Normal liver and background parenchyma of rats fed either of the diets for 10 &#xD;
weeks or 50 weeks showed weak telomerase activities. In contrast, markedly increased &#xD;
telomerase activities were found in hyperplastic nodules and HCCs. These results suggest &#xD;
that increased telomerase activity may be one biological phenomenon of preneoplastic &#xD;
lesions for evolution to HCCs in rat liver.</summary>

抄録: Our Department of Emergency and Critical Care Medicine in Nara Medical &#xD;
University opened in May, 1990. We evaluate and summarize current emergency patient &#xD;
transportation status and trends in the Chuwa district of Nara Prefecture on the basis of &#xD;
emergency transportation reports from several Emergency Medical Service (EMS) in this &#xD;
area. &#xD;
The number of outpatients in our department has decreased gradually. However, the &#xD;
ratio of severe patients has increased. The number of patients who visited the weekend- &#xD;
clinic and secondary emergency hospitals increased in Kashihara city. &#xD;
The number of emergency patients transported has been increasing according to the &#xD;
reports of EMS in the Chuwa district. However, the indistrict ratio (the ratio of patients &#xD;
transported to facilities inside the district which each EMS is responsible for) varied for the &#xD;
different EMS. The indistrict ratio increased in Chuwa and Nishikikatsuragi EMS, &#xD;
whereas it decreased in Sakurai and Kashiba-Koryo EMS. The time of patient transport &#xD;
tended to increase year after year in all EMS with the exception of Nishikatsuragi EMS. &#xD;
Although the in-district ratio increased, the time of patient transport tended to increase in &#xD;
the Chuwa district. The elongation of the addressing period to the receiving hospital, in &#xD;
addition to the increased time for emergency staff to reach the scene are considered to &#xD;
account for this situation. The increased time of patient transport in Sakurai and Kashiba &#xD;
-Koryo EMS was considered to be attributable to the decrease in the in-district ratio. An &#xD;
increase in the in-district ratio and a decrease in patient transport time were observed in &#xD;
Nishikatsuragi EMS every year, which was considered a model district for emergancy &#xD;
medicine.</summary>

抄録: We optimized the convenient nonradioactive polymerase chain reaction &#xD;
(PCR)-based single strand conformation polymorphism (SSCP) analysis using commer- &#xD;
cially available precast gradient polyacrylamide gels to detect point mutations in the p53 &#xD;
gene. The optimizatiOn was made by using 2-15% gradient gel whose running conditions &#xD;
were 2×Tris glycine or 1×TBE buffer, and 200 V for 2 hours at 4℃. &#xD;
Also, we screened 47 patients with various hematologic malignancies and detected p53 &#xD;
mutations in two patients with acute leukemia. Both of them showed a resistance to &#xD;
chemotherapy, whereas 20 Other patients with acute leukemia and without detectable &#xD;
mutation did not show such a resistance except for one single case. Mutations in the p53 &#xD;
gene could be closely associated with a resistance to chemotherapy in leukemias.</summary>

    <title>CLINICAL CHARACTERISTICS OF ISCHEMIC STROKE IN PATIENTS WITH DIABETES MELLITUS</title>

    <summary type="text">タイトル: CLINICAL CHARACTERISTICS OF ISCHEMIC STROKE IN PATIENTS WITH DIABETES MELLITUS

抄録: To investigate the clinical characteristics of ischemic stroke and its associa- &#xD;
tions with hypertension, obesity, hyperlipidemia, and diabetic microangiopathy in patients &#xD;
with diabetes mellitus, we estimated the average period between onset of diabetes and &#xD;
ischemic stroke, and the incidences of complications. A total of 544 patients with diabetes &#xD;
mellitus who were admitted to our hospital over the past 10 years were analyzed, forty of &#xD;
whom were diabetic patients who suffered ischemic stroke as diagnosed by clinical exami- &#xD;
nation. As controles, 40 subjects matched for sex, age, and duration of diabetes were &#xD;
identified, one for each patient with ischemic stroke. The average period between onset of &#xD;
diabetes and ischemic stroke was significantly shorter for patients with hypertension than &#xD;
for normotensive patients ; however, there were no differences in the length of this period &#xD;
between the groups with and without obesity or hyperlipidemia. Hypertension was present &#xD;
in 63% of patients with ischemic stroke, and the incidence of hypertension was significantly &#xD;
higher in the stroke group than in the control group. There was no difference between the &#xD;
groups with and without ischemic stroke with respect to the incidence of obesity, hyper- &#xD;
lipidemia, retinopathy and nephropathy. We conclude that diabetic patients with hyperten- &#xD;
sion appear to be at high risk for ischemic stroke.</summary>
